SMS/ISPS Form's - Completing a Form
Accessing the Forms Module
Open the SMS or ISPS Forms module from the navigation menu. You will see a list of available forms based on the templates uploaded in the Documentation Libraries. Select the form type you want to work with.

Reviewing Existing Forms
After selecting a form type, a grid will display all previously created forms, including their details and status. To start a new form, press the “New Form” button in the top right corner of the screen.

Creating a New Form
Enter a subject to help differentiate this form from others of the same type. Add a description explaining why the form is being opened if required. Finally press “Create” to save the new form instance.

Filling Out the PDF Form
Once created, press the “View Form” button to open the PDF fillable form in YMS. Complete the form as needed and press the “Save” button in the top right corner of the screen when the required changes have been made.

Updating the Form
Forms often need to be updated over time. You can reopen and edit the form as required using the “View Form“ button. Each time you make changes, save the form so a new revision is created. This maintains a complete audit trail showing which user made each update.

Completing & Closing a Form
YMS uses three system statuses for forms: Open, Completed and Closed. These statuses are set by entering a user and date in the Created, Completed and Closed inputs within the General Information section of the form’s Summary tab.

Open - Indicates the form is actively being worked on and may still be updated.
Completed - Indicates the user has finished entering information.
Closed - Indicates the form has been reviewed, verified and should no longer be changed.
